An unannounced renewal inspection was conducted on September 11th 2017 between 10:45am to 1:35pm. There were 50 children in the direct care of 10 staff members. Children were observed eating lunch, washing hands, being diapered, listening to stories at circle time, and preparing for nap time. Staff were observed to be consistently engaged with the children. The classrooms were clean, organized, and had plenty of age appropriate activities. Standard #: 22VAC40-185-130-B Description: Based on record review, 1 of 5 children's records reviewed did not have documentation of additional immunizations once every six months for children under the age of two. Evidence: Child # 4, born 12/7/2015, last had an updated immunization submitted 9/28/2016. Plan of Correction: Original Immunization record on file. Parent notified to update student immunization record and provide documentation by 9/22/17
Standard #: 22VAC40-185-70-A Description: Based on record review, 2 of 5 staffing records did not have all of the required documentation. Evidence: Staff #1 record does not contain education documentation. Staff #3 record only contained 1 of 2 required references. Plan of Correction: Staff #3 ? reference was completed prior to visit on 3/3/17, however form was updated with staff name and date, during visit. Staff #1 copy of Education Verification obtained on 9/11/17
Standard #: 22VAC40-185-510-E Description: Based on observation, medication reviewed was not labelled with the child's name. Evidence: The medication was stored with the child's other medication which was labelled with the child's name, but each medication must be labelled individually. Plan of Correction: Student medication bag was labeled with contents and pertaining documentation was present, however individual medication box was labeled during visit on 9/11/17
