Comments:
A Subsidy Health and Safety Inspection was conducted on September 22, 2017, to review supplemental health and safety requirements. Licensing inspector arrived at 6:05am and departed at 8:40 am to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 25 school age children present in care with four staff members. Children were observed arriving to the program, playing with various tabletop activities and participating in a share and show activitiy. The sample size consisted of four staff records and five children's records. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on record review, in one of five children's records reviewed, the vendor did not ensure that there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: A review of the record for Child #1 confirmed that the child had a diagnosed food allergy and there was not a written allergy care plan with instructions from a physician.

Plan of Correction: Parent has been contacted and informed that we are in need of an allergy plan.

Standard #: VENDSUB-000-006-B-3
Description: Based on record review, in five of five children's records reviewed, the vendor did not ensure that all required parent contact information was in each child's record. Evidence: The employment addresses of the parents for Child #1-Child #5 were not indicated in the records.

Plan of Correction: I will contact the parents immediately about putting in their work addresses.

Standard #: VENDSUB-000-031-A
Description: Based on review, the vendor did not ensure that the center had a written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation, relocation, lockdown and shelter-in-place procedures. Evidence: The written emergency preparedness plan did not address the following required procedural components: 1. Evacuation and Shelter-in-Place Procedures: Accommodations or special requirements for children with special needs to ensure their safety during evacuation or relocation. 2. Lockdown: Methods to alert staff and emergency responders; Methods of communication with parents; Accommodations or special requirements for children with special needs to ensure their safety during evacuation or relocation; Procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child.

Plan of Correction: I will discuss with the staff regarding the above components and come up with a plan of how to implement those items.
