A Subsidy Health and Safety Inspection was conducted on September 25, 2017 to review supplemental health and safety requirements. The Licensing Inspector arrived at 6:00am and departed at 8:00am to conduct a Subsidy Health and Safety Inspection. At the time of the tour, there were 12 school age children present with three staff members. The licensing inspector observed the children arriving to the program, playing with various tabletop toys and blocks and talking freely amongst themselves. The sample size was five children's records and two staff records. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-006-B-3
Description: Based on record review, in five of five children's records reviewed, the vendor did not obtain all required parent contact information. Evidence: The parent's work addresses were not documented for the records of Child #1-Child #5.

Plan of Correction: We will talk to all parents about getting us work addresses.

Standard #: VENDSUB-000-012-E
Description: Based on review, the vendor did not ensure that all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval: Current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. The training shall include an in-person competency demonstration; and Current certification in first aid. Evidence: There was no documentation of CPR and first aid training for Staff #1.

Plan of Correction: Currently working on trying to find a trainer so that the staff member can get the required training.
