The licensing inspector conducted an unannounced renewal inspection Monday, September 25, 2017 from approximately 10:15am to 3:00pm. The following ratios were observed with a total of 63 children present during the inspection: Infants - 7 children with 2 staff Toddlers - 4 children with 1 staff Toddlers/Twos - 8 children with 2 staff Twos/Threes - 15 children with 2 staff Preschool - 10 children with 1 staff Pre-K - 19 children with 2 staff School age children were not present during the inspection. The children and staff were engaged in a variety of activities when the licensing inspector arrived. Infants were observed playing on the floor with their teachers, sleeping in cribs and being fed. The Toddlers were observed being read to and resting at naptime. The Twos were observed washing their hands and eating lunch. The Preschool children were observed building with blocks and legos, playing on the easel, looking at books and participating in a dance class. The Pre-K children were observed playing outside and preparing for lunch. Lunch consisted of turkey and cheese wraps, green beans, pears and milk. All areas of the center including classrooms, hallways, playgrounds, and bathrooms were inspected. The facility was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, injury prevention plan, daily sheets, daily attendance, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The center's first aid kit and emergency supplies were inspected. Medication is administered and medication and authorization forms were reviewed. Transportation is provided and the bus and transportation documents were inspected. Last documented health inspection: 04/25/2017 Last documented fire inspection: 03/03/2017 During the inspection, ten children's records and ten staff records were reviewed. The risk profile tool will be used for the licensing recommendation by the licensing department. The renewal application was received prior to the expiration of the current license. Standard #: 22VAC40-185-60-A Description: Based on review of 10 children's records, two records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the last documented confirmation of up-to-date information in the record for Child #4 was dated as 07/30/15 and the record for Child #8 was dated as 01/18/14. Children's records are required to have at least an annual review to ensure the child's record is up to date. Plan of Correction: We will reach out to the families and have them review the information in the record.
