Licensing Inspector arrived at 9:45 AM and departed at 12:00 PM. At the time of the inspection, there were nine children, ages infant through preschool, in care with three staff. Children were observed participating in educational activities. Center documents and medication were also reviewed. The sample size was four staff and four children's records. Violations were cited in the areas of administration, physical plant and special care provisions & emergencies.
Standard #: 22VAC40-185-140-A Description: Based on a review of four children's records, it was determined that the facility did not ensure that each child shall have a physical examination within 30 days of enrollment. Evidence: The record for child #2, date of enrollment 9/26/16, did not have a completed physical examination on file. Plan of Correction: Facility will request physical from the parent. All children's files will be checked to ensure that they are complete.
Standard #: 22VAC40-185-60-A Description: Based on a review of four children's records, it was determined that the facility did not ensure that all required information was present in each child's record. Evidence: The record for child #4 did not include addresses for the two required emergency contact persons. Plan of Correction: Addresses will be obtained and placed in the child's file. All other files will be checked to ensure that they are complete.
Standard #: 22VAC40-185-280-B Description: Based on inspection of the physical plant, it was determined that the facility did not ensure that all hazardous substances were kept in a locked place. Evidence: There was an unlocked cabinet in the back bathroom near the infant room that contained cleaning supplies. Plan of Correction: Cabinet was locked during the inspection. Staff will be reminded to keep cabinets containing chemicals locked.
Standard #: 22VAC40-185-510-G Description: Based on review of medication, it was determined that the facility did not ensure that all requirements were met in order to administer prescription medication. Evidence: The record of administered medication indicated that Ritalin was administered to a child all summer by a staff member without medication administration training (MAT). Plan of Correction: No medication is currently being administered. Facility management will ensure that staff obtain MAT prior to administering medication.
