Comments:
Today was an unannounced monitoring inspection that began at 9:30am and ended at 11:20am. Today there were 8 children with four staff including the center Director and assistant. During today's inspection, staff and children's records were reviewed, file drill and evacuation logs were reviewed, the physical plant was inspected. The children were observed working in small groups and having one-on-one interactions with some of the teachers. All staff interactions with the children were positive. Standard #: 22VAC40-185-60-A
Description: Based on file review, the children's records did not contain all of the required information. Evidence: Two out of four children's files did not contain an annual update of contact information. Child #1 date of enrollment was 9/19/2016. Child #2 date of enrollment was 8/29/2016.

Plan of Correction: I will ask the parents to update the children's files.
