22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal visit was conducted on 9/19/2017. The inspection began at 10 A.M and concluded at 1:30 pm. There were twenty nine children present with eight direct care staff and the Program Director was also present. During the inspection seven staff's and five children's records were reviewed. All three outdoor playgrounds were viewed, hand washing was observed, infant care was observed, all four classrooms were observed, a sample of injury reports were reviewed, emergency supplies were viewed, lunch time was observed, nap time was observed, and some policies and procedures were reviewed. The plan of correction will be due in the Licensing office on Tuesday September 26, 2017.
Standard #: 22VAC40-185-270-A Description: Based on observation of the preschool play ground, the center failed to ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. The inspector observed that the under covering underneath the resilient surfacing on the preschool playground was exposed in two places and caused a tripping hazard for the children in care. 2. Under the picnic area there was the start of two bee hives. This area is used by the children when playing outside. 3. The school age door frame that leads to the playground had rotten wood and the wood has started to splinter and could snag the skin or clothes of the children. 4. The plastic cover of the basement windows was cracked and posed a hazard to staff and children. Plan of Correction: ? #1 The PD will submit a maintenance request to add more resilient surfacing on the Preschool playground (9-29-17) ? #2 The PD will contact maintenance about the bee hives (9-25-18) ? #3 The PD will submit a maintenance request to replace the door of the school age leading to outside (9-29-17) ? #4 The PD will submit a maintenance request to fix the basement windows (9-29-17)
Standard #: 22VAC40-185-330-B Description: Based on observation the center failed to ensure that all playground equipment had the required amount of resilient surfacing. Evidence: 1. Immediately under the swings there was very little resilient surfacing remaining. Dirt and ground was able to been seen from where the resilient surfacing has been kicked out. Plan of Correction: ? The PD will submit a maintenance request for resilient surfacing to be added under the swings on Preschool/Afterschool playground
Standard #: 22VAC40-185-420-E-3 Description: Based on review of five children's records, the center failed to ensure that all children had an annually confirmation that the child's record is up to date. Evidence: 1. Child # 2 did not have the 2017 annual update. Child # 2 most recent up date was 8/31/2016. 2. Child # 4 did not have the 2017 annual update. Child # 4 most recent up date was 8/31/2016. 3. Child # 5 did not have a 2016 and 2017 annual update. Child #d 5 most recent update was 9/24/2015. Plan of Correction: ? The PD will ensure that annual updates are done and they will go out to all families on 10-1-17 and will be completed by (10-16-17)
Standard #: 22VAC40-185-560-F-4 Description: Based on observation of lunch time, the center failed to ensure that all food offered to children three years of age or younger may not be choke hazards. Evidence: 1. The red bears class room was served corn dog nuggets that were not diced. The youngest child in the grouping was 19 months old. Corn dog nuggets are considered a choke hazard. Plan of Correction: ? The PD will remind Staff that the food offered to 3 years and younger that the food must be diced up to meet the choke hazard. (9-19-17)
