Reminder: Two shelter-in-place drills are required each calendar year.

Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:30am - 12:00pm. There were twenty eight preschool children in care: thirteen children with two staff in classroom #1 and fifteen children with two staff and one volunteer in classroom #2. The children were observed making bracelets, at circle time, playing in the centers and preparing for lunch. Six children's records were reviewed. Violations from the previous inspection were also reviewed and all had been corrected. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain an outside play area in a safe condition. Evidence: 1. The inspector observed the outside play areas. There is black plastic protruding through the pea gravels creating tripping hazards on the play area located behind the center.

Plan of Correction: The teacher will complete a request for maintenance today. The children will be monitored closely when playing in this area.

Standard #: 63.2-1809-A
Description: Based on record reviews, the center failed to obtain proof of a child's identity and age. Evidence: 1. The inspector reviewed six children's records. One of six records reviewed (child record #6) did not contain proof of child's identity and age and child's date of enrollment was 08/29/17.

Plan of Correction: The teacher will remind the child's parent again that this information is needed for the child's record.
