An unannounced complaint inspection was conducted on September 20, 2017, from approximately 2:55 p.m. to 5:10 p.m. There were 85 children present and 138 children enrolled in the program. The inspection was in response to a complaint that was received in the Valley Licensing Office on September 15, 2017. The complaint was in regard to supervision of children and the physical plant. The preponderance of the evidence gathered during the investigation did support one of the allegations and the complaint disposition was, therefore, "valid." The violation issued was noted on the violation notice. There were three staff members interviewed, two staff members' records and two children's records reviewed. Description: Based on observation, staff interviews, and verification from the director, the center failed to keep hazardous substances in a locked location which prevented access by children. Evidence: 1. On 09/14/2017, two children ran ahead of three staff members and the rest of the children as the group walked in the building from the playground. The children went into the coat room at the end of the hallway. The coat room was not locked and on open shelves in room were 7 gallons of paint, a spray bottle of Spartan Spray Buff, and several containers consisting of Spartan Stainless Steel Cleaner, Damp Mop Detergent Concentrate, NABC Disinfectant Bathroom Cleaner, Spar San Q, and step Down Finish Liquidator. 2. The director verified the cleaning supplies and paint were accessible to the two children as they entered the coat room.

Plan of Correction: The program director put in a work order with maintenance on 09/20/2017. On 09/26/2017, a lockable cabinet was installed in the coat room and all of the hazardous substances are now being stored in the locked cabinet.
