An unannounced Subsidy Health and Safety Inspection was conducted on September 28, 2017 from approximately 6:40 a.m. until 9:00 a.m. There 25 children in care and 3 staff present with children. The record review sample consisted of 5 children's records and the on site required documentation for 7 staff records. Children were observed playing in centers to include board games, homeliving, cars and dolls. Children were also observed eating breakfast and playing large group games. The results of the inspection were reviewed with the center director. Areas of non-compliance are noted on the violation notice. Standard #: VENDSUB-000-006-B-11
Description: Based on documentation review and staff interviews, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on September 28, 2017, the record for child #4 has documentation of a diagnosed food allergy (eggs,peanuts and shellfish) and there is not a written allergy care plan.

Plan of Correction: The center director will contact the parent to provide the allergy plan for the child.

Description: Based on observation and interviews, the vendor did not ensure all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval CPR and first aid training. Evidence: During the inspection on September 28, 2017, the records for staff #3, #4, #5 and #7 did not have documentation of obtaining CPR and first aid training. The cetner director reported that all staff have been employed for more than 30 days.

Plan of Correction: The center director will communicate with the training coordinator and have the classes scheduled for the staff.

Standard #: VENDSUB-000-031-B-1
Description: Based on documentation review and staff interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an evacuation, shelter-in- place and lockdown. Evidence: During the inspection on September 28, 2017, there were not reunification procedures documented for evacuation, shelter-in-place and lockdown.

Plan of Correction: The center director will update the document to state that parents/authorized pick up will have id and will sign said child out from the program at pick up.
