22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 4:00pm and 5:30pm. At the time of the tour there were 24 children in care with 3 staff present. Children were observed eating snack, doing bathroom break, stretching and doing homework. Check-out was also reserved. A sample of 3 staff and 4 children's records were reviewed. The center did not have any medication at the time of inspection.
Standard #: 22VAC40-185-60-A Description: Based on a record review the center failed to ensure each child's record contained the required information. Evidence: 1. Out of four children's records reviewed, four did not have documentation of proof of identification. 2. Child #4 did not have documentation of a phone number for the child's physician. Plan of Correction: The director is working on obtaining all birth certificates.
Standard #: 22VAC40-185-560-F Description: Based on observation the center failed to ensure all requirements were met when serving snacks. Evidence: The school-age children were observed eating baked potato chips for afternoon snack with no drink. After snack the children were sent to the water fountain for a water. The menu stated the children were to be served Cheez-its and milk. Plan of Correction: The center did not have cheez-its and the milk was in the kitchen. The director will ensure staff serve milk with snack.
