A Subsidy Health and Safety inspection was conducted on 09/28/17 to review supplemental health and safety requirements. Licensing Inspector arrived at 4:00pm and departed at 5:30pmpm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 24 children in care with 3 staff present. Children were observed eating snack, doing bathroom break, stretching and doing homework. A sample of 3 staff and 4 children's records were reviewed. The center did not have any medication at the time of inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-006-B-11 Description: Based on a record review and interview with staff the vendor failed to ensure there was a written written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child #4's physical stated she had an allergy to seafood and peanuts. The vendor did not have a written allergy plan for child #4. Plan of Correction: The director will obtain a written allergy plan signed by the child's doctor to address child#4's food allergies.
Standard #: VENDSUB-000-031-A Description: Based on a record review the center failed to ensure the written emergency preparedness plan included all required components. Evidence: The center's written emergency plan did not include reunification plans for the following scenarios: evacuation, shelter-in-place and Intruder. Plan of Correction: The center will include these components in the emergency plan and ensure they are site specific.
