A Subsidy Health and Safety inspection was conducted on September 22, 2017 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A Violation was found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11
Description: Based on record review of a child's file, it was determined the center has not obtained a physician's written allergy care plan for each child who have recorded food allergies. Evidence: 1. Staff stated they have a list of children who have recorded food allergies. Inspector reviewed one child's file with a listed food allergy but there was no written instructions from a physician regarding steps to take in the event of a suspected or confirmed allergic reaction.

Plan of Correction: We will contact the parents and obtain plans.
