A Subsidy Health and Safety inspection was conducted on 09/29/2017 from 2:03pm to3:45pm to review supplemental health and safety requirements. The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-3 Description: Based on record review, four of five children's records did not have complete information. Evidence: 1. Four children's records had the custodial parent's place of employment and phone number, but did not have the address. Plan of Correction: I will ask the parent's for the information.
