T/A was provided in the area of primary care groups.

Comments:
An unannounced complaint inspection was conducted in response to a complaint that was received by the licensing office on 9/22/17. Interviews were conducted with the staff relating to allegations of inappropriate discipline of a child (verbal and physical) and a lack of proper supervision of children. Surveillance video was also viewed during the times the alleged incidents occurred. The information gathered during the investigation does not support the allegations, so the complaint is determined to be ?not valid.? Although during the investigation it was found that information was missing from an injury report from an incident that was was unrelated to the complaint and a violation was issued. Description: Based on the review of the center records and interview. The provider failed to complete a written record of the time that the parents were notified of the children's injury on the incident report. Evidence: 1 ? The incident report for child #1(dated for 09/21/2017) did not include a written record of the time that the parents were notified of the children's injury. 2 ? The provider confirmed that incident report was not entirely completed and time was not documented on the incident report.

Plan of Correction: The provider will confirm that when an incident occurs that requires an accident/incident report that the documentation on the report will be entirely completed on the day that the incident occurred.
