A subsidy Health and Safety Inspection was conducted on September 28, 2017 to review supplemental health and safety requirements. Licensing Inspector arrived during the before-care portion of the program arriving at 6:00am and departed at 8:00am to conduct a Subsidy Health and Safety Inspection. The children were observed arriving to the program, talking freely amongst themselves and doing various tabletop activities. There were 32 children present with two staff members. Five children's records were reviewed during this inspection. Two staff records were reviewed during the inspection. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-006-B-3 Description: Based on record review, in four of six children's records reviewed, the vendor did not ensure that all required parent contact information was maintained in each child's record. Evidence: There was no documentation of both parent's work addresses in the record for Child #1, Child #3, Child #5 and Child #6. Plan of Correction: The parents will be contacted and work addresses will be added to each child's record.
