An unannounced Renewal study was conducted today after school. There are 6 staff and 72 children present. 6 adult and 7 children's files were reviewed. See violation notice form missing items. Children were observed playing, washing hands, eating snack and enjoying the playground. A Subsidy Health and Safety Inspection (SHSI) was conducted jointly with this inspection. Standard #: 22VAC40-185-70-A Description: Based upon a review of records, one staff file is incomplete. Evidence: Staff A does not have an emergency contact person on file. Plan of Correction: Staff A is a substitute. I will have the subs fill out contact information forms.
Standard #: 22VAC40-185-280-B Description: Based upon direct observation, hazardous materials were not stored locked. Evidence: Activated charcoal was stored in a cabinet above the sink in room 2. Plan of Correction: It was moved to a locked location during the inspection. It will kept locked.
Standard #: 22VAC40-185-510-G Description: Based upon a review of medications, prescriptions do not have a label attached. Evidence: 2 of 6 medications do not have prescription labels attached. (Child 1,2) Plan of Correction: I will notify the parents to bring in medication with labels attached.
Standard #: 22VAC40-185-510-J Description: Based upon observation, one medication was not stored locked. Evidence: Child 2's medication was stored in an unlocked backpack under the staff desk. Plan of Correction: I will store the medication locked.
