A Subsidy Health and Safety inspection was conducted on 10/02/2017 to review supplemental health and safety requirements. There are 6 staff and 72 children after school today. 6 staff files and 7 children's files were reviewed. Children were observed playing washing hands, eating snack and playing outdoors. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. LI was present today from 3:30 PM - 6:15 PM
Standard #: VENDSUB-000-012-B Description: Based upon a review of records, one staff file is incomplete. Evidence: Staff A does not have documentation of having completed the preservice training. Plan of Correction: Staff A will obtain the documentation.

Standard #: VENDSUB-000-031-B-1 Description: Based upon a review of records, staff records are incomplete. Evidence: The emergency preparedness plan has not been updated, to include reunification, lockdown procedures. Plan of Correction: Center supervisor will update the plan.
