A complaint was received on 7/28/17 regarding potential violations of licensing standards in the areas of: supervision, electrical outlets not covered, parental notification of an incident, staff training, staff orientation, qualifications of staff. An investigation was initiated by the inspector at the facility on 7/31/17. Staff records were reviewed and the child care director was interviewed. Follow-up interviews were conducted with the local department of social services on 8/2/17 and 8/3/17. An additional inspection at the facility was made on 8/21/17 to review records and other documentation. The inspection was completed and emailed to the provider on 9/19/17. Based on the observations, record review, and interviews, allegations of the complaint were substantiated and violations of the Standards for Licensed Child Day Centers were cited.
Standard #: 22VAC40-185-290-3 Complaint related: Yes Description: Based on interviews with staff, the center failed to ensure that electrical outlets had protective covers. Evidence: 1. Staff indicated through interviews that the outlets in the gymnasium did not have protective covers on them on the date of the incident, 7/27/17. Plan of Correction: All outlets have been covered in the gym.
Standard #: 22VAC40-185-550-M Complaint related: Yes Description: Based on documentation review, the center failed to ensure that documentation of all required information was made on an injury report. Evidence: 1. The "BAFY Kids' Club First Aid and Sickness Report" for child #1 dated 7/27/2017 was reviewed. The report did not have documentation of the method or the time of notification to the parent. Plan of Correction: All injury reports will be fully completed. The child care director will discuss at next staff meeting planned for August 5, 2017.
