A physician plan of action for allergies needs to include a plan for suspected reaction and/or confirmed reaction for each food item.
A Subsidy Health and Safety inspection was conducted on October 2, 2017 to review supplemental health and safety requirements. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Standard #: VENDSUB-000-006-B-11 Description: Based on record review of 5 children's files that have listed allergies, the center has not obtained written instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected and/or confirmed allergic reaction for all of them. Evidence: 1. Child 1, 2, 3, and 4 have listed food allergies but there are no written plan of actions from physicians. 2. Child #1 has a long list of foods but these foods are not on the physical report. The report has nuts but not the different shellfish listed. Plan of Correction: Not available online.
