Comments:
An unannounced monitoring inspection was conducted today from 7:00am-8:45am. There were 52 school-agers directly supervised by 3 staff. The physical plant, 5 staff records, 5 children? records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to care, participating in group play, individual activities and eating snack. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. If you have any questions regarding this inspection, please contact the Licensing Inspector. Standard #: 22VAC40-185-130-A
Description: Based on review, the facility failed to obtain documentation of an immunization record before the child can attend the center. Evidence: Reviewed 5 children's records and found that 1 did not have documentation of an immunization record.

Plan of Correction: I will go to the main office today and make a copy of the child's immunization record.

Standard #: 22VAC40-185-140-A
Description: Based on review, the facility failed to ensure that each child has documentation of a physical examination. Evidence: Reviewed 5 children's records and found that CH5 did not have documentation of a physical exam.

Plan of Correction: I will go to the main office today and make a copy of the child's physical examination.

Standard #: 22VAC40-185-150-B
Description: Based on review, the facility failed to ensure that each child's immunization record is signed by a physician, their designee or an official of a local health department. Evidence: Reviewed 5 children's records and found that CH4 did not have documentation of a signed immunization record.

Plan of Correction: I will go to the main office today and make a copy of the child's signed immunization record.

Standard #: 22VAC40-185-260-A
Description: Based on review, the facility did not have documentation of an annual fire inspection report. Evidence: Reviewed and identified through the facility's records that there was no documentation of an annual fire inspection.

Plan of Correction: Will get a copy from the main office as soon as children are released to the school, this morning.

Standard #: 22VAC40-185-510-C
Description: Based on review, the facility failed to ensure that they were following their policy of returning outdated medications. Evidence: Observed that there was expired medication (albuterol) on site and had not been returned to the family.

Plan of Correction: Will contact parents this afternoon and ask them to bring in new medication ASAP.
