Discussed with director changes in background checks requirements, medications.
An unannounced renewal inspection was conducted from 9:15am-12:30pm. During the same day, a Subsidy Health and Safety inspection was conducted to review supplemental health and safety requirements. 27children were in care with 8 staff members present. Children were observed during story time, gross motor activities and outdoor play time. The teachers were seen interacting with the children during their activities. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-270-A Description: Based on observation, inside and outside areas of the center were not maintained in a clean, safe and operable condition. Evidence: 1. A broken table was found on the playground. The table had broken parts. 2. A broken toy grocery cart was found on the playground, outside of a classroom. 3. 2 rusty nails were found on the concrete sidewalk of the playground and the smaller playground for the younger children. 4. The metal doors enclosing the air conditioning units were not locked, making the units accessible to children. 5. The home living kitchen in the Two's classroom had missing pieces, leaving open holes which could cause a potential entrapment hazard for the children's fingers. 6. In the preschool classroom two soft red chairs had tearing and holes. Plan of Correction: Table and broken toys were disposed off on 9/22. Rusty nails were thrown out by licensing during inspection. Facilities contacted and a work order put in place for the metal doors around a/c unit. Home living kitchen was plugged in by facility. Chairs were repaired per licensing suggestion.
Standard #: 22VAC40-185-280-B Description: Based on observation, cleaning materials were not stored in a locked place and were accessible to children. Evidence: Cleaning solution bottles were stored in a cabinet in the school age room. The lock to the cabinet was broken, making the cleaners accessible to children. Plan of Correction: Lock replaced on the spot during inspection.
Standard #: 22VAC40-185-520-B Description: Based on observation, sunscreen was found unlabeled and accessible to children. Evidence: 1. A bottle of sunscreen was found in a child's cubby, within reach of children. 2. The sunscreen was not labeled with the child's name. Plan of Correction: The sunscreen was moved to an inaccessible area during the inspection.
