Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on October 4, 2017 from 2:30p.m. through 5:30 p.m. On this date, there were twenty-four (24) children in the care of three (3) staff members. Five (5) children's records and three (3) staff records were reviewed and updated. Currently no medication is being administered. Areas observed were arrival, hand washing/bathroom procedures, snack, indoor games, transition, and homework. All interactions between staff and children were appropriate and the children seemed comfortable with the daily routine. Standard #: 22VAC40-191-40-D-1-A Description: Based on record review the facility failed to have a sworn statement, criminal history record check and a search of the central registry for one new Board Officer. Evidence: 1. Record review did not show documentation of a sworn statement , criminal history record check and a central registry search for Board Officer # 3. 2. The director confirmed there was no documentation any background checks Board Officer # 3. Plan of Correction: All required background checks will be submitted for the new Board Officer. This licensing inspector will be notified once they are received.
