Standard #: 22VAC40-185-60-A
Description: Based on record review, 2 of 2 children's records were missing information. Evidence: Child #1 was missing a second emergency contact address and Child #2 was missing both emergency contact addresses.

Plan of Correction: Center Management will request parents complete the required addresses.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the janitor hall closet is not locked and contains cleaning materials. Evidence: The hall closet is between the two bathrooms the program uses daily and the door was unlocked allowing access by children to cleaning materials.

Plan of Correction: Center Management will remind the custodial staff that the door must remain locked during preschool hours.

Standard #: 22VAC40-185-330-B
Description: Based on observation, an inadequate amount of resilient surfacing material is under the two swings on the playground. Evidence The area under the two swings does not have the minimum 6 inches of mulch to comply with minimum safety standards for fall zones.

Plan of Correction: A maintenance request will be submitted for additional mulch to go under the swings.
