An unannounced inspection was conducted from 10:15am to 12:25pm. At the time of entrance, there were 43 children present with 8 staff present and 1 administrator. The inspector observed children participating in instructional time, free play and infant activities, lunch, toileting and hand washing. A sample size of 6 children's records and 6 staff records were reviewed. No medications are being administered and there were no injury reports since the last inspection. Areas of noncompliance are identified in the Violation Notice and were discussed with the Director at the conclusion of this inspection.
Standard #: 22VAC40-185-130-B Description: Based on record review it was determined that the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: The record for child #6(DOB 8/28/16) did not contain updated immunization documentation. The last documentation available was dated 3/22/17. Plan of Correction: Center will request updated documentation from the parent.
