A Subsidy Health and Safety inspection was conducted on October 6, 2017, to review supplemental health and safety requirements. At arrival to the facility, three staff were present with 19 children. Licensing inspector arrived at 8:20am and departed at 12:45pm. Additional staff arrived for support and administrative purposes. Children were observed playing with table top activities. Infants were observed crawling and exploring, feeding and during tummy time. Five children's records and five staff record were reviewed on this date. A licensing insepction was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-005-B
Description: Based on review, the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: There was no documentation of the departure of two school age children on the date of the inspection.

Plan of Correction: Director will be sure that children will be checked out prior to departing to school.

Standard #: VENDSUB-000-012-E
Description: Based on record review, in one of five staff records reviewed, the vendor did not ensure that all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval: Current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. The training shall include an in-person competency demonstration; and Current certification in first aid. Evidence: There was no documentation of CPR and first aid training in the record for Staff #1 (hire date 8/15/17).

Plan of Correction: The staff is scheduled for CPR/First Aid training tomorrow.

Standard #: VENDSUB-000-031-A
Description: Based on review, the vendor did not ensure that the written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation, relocation, lockdown and shelter-in-place procedures contained all required procedural components. Evidence: The center's written emergency preparedness plan did not address the following procedural components: 1. Evacuation and Shelter-in-Place Procedures: Procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child. 2. Lockdown Procedures: Methods to alert staff and emergency responders; Methods to secure the facility and designated lockdown locations; Methods to account for all children in the lockdown locations; Methods of communication with parents and emergency responders; Accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during lockdown; and Procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child.

Plan of Correction: Due to lack of communication from DSS as to specific guidelines for emergency preparedness we did not have appropriate information. All documentation will be updated immediately.
