An unannounced monitoring inspection was conducted from 9:00 am - 1:15 pm. There were 82 children in care being supervised by 14 staff members. The children were observed participating in structured center play, circle time activities, and outdoor play. Lunch served today is hot turkey sandwich, mashed potatoes, green beans, whole-grain bread, and milk. Ten children's records were reviewed and in compliance. Seven staff records were reviewed. Four medications were reviewed. There are currently 4 staff on site who are MAT (Medication Administration Training) certified. The most recent emergency escape drill was conducted 09/18/2017 and the most recent shelter in place drill was conducted 09/25/2017. Standard #: 22VAC40-185-160-C Description: Based on review of seven staff records it was determined that not all TB (Tuberculosis) screenings were repeated every two years. Evidence: The most recent TB Screening on file for Staff B was dated 05/26/2015. Plan of Correction: Staff B will locate the most updated TB screening and will submit the results to administration.
Standard #: 22VAC40-185-270-A Description: Based on observation of the two classroom it was determined that not all areas or equipment were maintained in safe condition. Evidence: The plastic cot cover in the two year old classroom was cracked on all four corners with sharp edges exposed. Plan of Correction: The cot lid was disposed of during the inspection.
Standard #: 22VAC40-185-500-B Description: Based on observation of diapering in the Toddler classroom it was determined that not all procedures for diapering were followed. Evidence: 1. Staff A disposed of a diaper in a trash can that was not foot operated. 2. Staff A did not clean the diapering surface after a diaper change. Plan of Correction: All staff will receive re-training in diapering procedures.
Standard #: 22VAC40-185-510-C Description: Based on observation four medications it was determined that not all procedures for administering medication were followed. Evidence: Child A (Age 2) had a long term medication authorization form that was not accompanied by a physician signature. Plan of Correction: Administration will request the physician signature from the parents of Child A.
