Licensing Inspector arrived at 9:30am and departed at 12:30pm to conduct a monitoring Inspection. At the time of the tour there were 45 children in care with 10 staff present. A sample of 4 children and 4 staff records were reviewed. The center did not have any medication at the time of inspection. Children were observed playing in centers, on the playground and getting ready for a story. Infants were observed playing on the floor with staff. School-age children were also observed entering the building after being picked-up from school.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe condition. Evidence: In the preschool bathroom there were screws in the wall above the toilets that were rusted and the rust was staining the wall. The hardware at the base of the toilets were also rusted.

Plan of Correction: Maintenance will be contacted to remove the rust.

After the diaper was changed the staff member did not wash her hands after, nor she wash her hands before changing the next child.

Plan of Correction: During the inspection the Assistant Director reminded staff to wash her hands before and after changing a diaper. Administration will retrain staff on proper diapering and hand washing procedures.

Standard #: 22VAC40-185-500-B

After the diaper change the staff was observed discarding the table pad and gloves in the foot operated trash can by opening it with her hands. The foot pedal was not used.

Plan of Correction: During the inspection the Assistant Director reminded staff to use the foot pedal when discarding diapers and soiled debris. Administration will retrain staff on diapering.
