A sample of 4 children and 4 staff records were reviewed. The center did not have any medication at the time of inspection. Children were observed playing in centers, on the playground and getting ready for a story. Infants were observed playing on the floor with staff. School-age children were also observed entering the building after being picked-up from school.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe condition. Evidence: In the preschool bathroom there were screws in the wall above the toilets that were rusted and the rust was staining the wall. The hardware at the base of the toilets were also rusted. Plan of Correction: Maintenance will be contacted to remove the rust.
