A Subsidy Health and Safety inspection was conducted on this date to review supplemental health and safety requirements. A total of 6 children's records and 1 staff record was reviewed. 1 staff on site which did not have a staff record maintained. All medications reviewed, evacuation drills, emergency plan and attendance record were reviewed. Children were observed entering the programming area, settling into small groups and completing homework, table top activities such as drawing, legos and setting up for snack which consisted of juice and cinnamon rolls. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Description: Based on child record review, one child has a diagnosed food allergy. An allergy care plan did not include instructions from a physician. Evidence: Child #6 has a food allergy and is prescribed a medication. The allergy care plan maintained was not signed by a physician to include instructions to be taken in the event of a confirmed or suspected allergic reaction.

Plan of Correction: Will request a signed action plan by the physician from the child's parent.

Description: Based on staff record review, one staff did not have documentation of Pre Service training completion. Evidence: Staff #2 is a substitute. No documentation found that Pre Service training was completed.

Plan of Correction: Staff will maintain all training and certifications with them at each center.

Description: Based on staff record documentation, one staff does not have documentation of CPR/First Aid. Evidence: Staff #2 is a substitute for this center. No verification of CPR/First Aid found.

Plan of Correction: Staff will carry all training certificates with them at each center they are substituting at.
