Comments:
Today an unannounced monitoring inspection was conducted starting at 2pm. There were 25 children present today with 2 staff. Children were observed entering the programming area from their classrooms, participating in table top actitivies which included homework, lego and drawing. Handwashing procedures were observed before snack which consisted of juice and cinnamon rolls. All medications with authorizations were reviewed. MAT (medication administration training) training and first aid/CPR certifications were checked today. Staff records are reviewed annually in the administrative office. This inspection, including the exit interview ended at 4:15pm. Please email me at lisa.hudson@dss.virginia.gov with any questions or clarifications needed.

Description: Based on review of staff records, one staff record was not maintained. Evidence: Staff #2 is a substitute. No documentation found for this staff to include CPR/First Aid certifications, emergency contacts.

Plan of Correction: Staff will bring copies of all required information with them at each school

Standard #: 22VAC40-185-510-E
Description: Based on review of all medication on site, one medication was found not labeled wit the child's name. Evidence: Child #7's benadryl was not labeled with their name.

Plan of Correction: This medication will be labeled today.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation of the physical plant, a required posting was not maintained. Evidence: The most recent licensing violation notice dated 12/20/16 was not posted as required.

Plan of Correction: We will post today's violation notice.
