Comments:
A Subsidy Health and Safety inspection was conducted on 10/11/17 to review supplemental health and safety requirements. Licensing Inspector arrived at 11:00am and departed at 1:00pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 68 children in care with 16 staff present. A sample of 6 children and 6 staff records were reviewed in addition to 2 medications. Children were observed during nap. Infants were observed playing on the floor with staff and being fed. School-agers were observed doing an activity with staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on a record review the vendor failed to ensure the written emergency preparedness plan included all required components. Evidence: The center's written plan did not include reunification plans for an evacuation, lock down or shelter-in-place.

Plan of Correction: The director will add reunification specific to each procedure.
