An unannounced monitoring inspection was conducted from 9:09 am-1:15pm. At the time of entrance, 35 children were in care with 9 staff members present. Children were observed during circle time in two classrooms, practicing their writing skills, and engaged in group activities. Infant care was observed(feeding, diapering, sleeping and playing). The center is clean and organized. A selection of staff and children records, and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1. In the giraffe classroom and in the cubs classroom, an odor eliminator was found in each bathroom on top of the toilet. 2. In the ducks classroom, an odor eliminator was found on a low shelf in the classroom. Plan of Correction: I removed it immediately and will find a compliant holder.
Standard #: 22VAC40-185-500-B Description: Based on review, the diapering area did not meet all requirements. Evidence: 1. One diaper pad that was located in the infant room had a rip in it and does not meet the requirement of being a nonabsorbent surface. Plan of Correction: It was replaced immediately.
Standard #: 63.2-1720.1-B-3 Description: Based on record review, staff records did not include all the required information. Evidence: 1. Staff B began employment on 03/13/2017 and does not have documentation that a central registry was sent out for the state the staff member resided in the past five years. 2. Staff D began employment on 03/07/2017 and does not have documentation that a central registry was sent out for the state the staff member resided in the past five years. 3. Staff E began employment on 10/05/2015 and does not have documentation that a central registry was sent out to the State the staff member currently resides in. Plan of Correction: Now that we are aware, we will send out as soon as possible.
