Comments:
An unannounced renewal inspection was conducted from 8:30 am-1:00pm. At the time of entrance, 67 children were in care with 15 staff members present. Children were observed during circle time, reading books, singing, snack time, infant care, hand washing and engaged in group activities. A selection of staff and children records, medications and other documents were reviewed. All staff had CPR and First Aid training. Area s of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center inside and outside were not maintained in a safe and operable condition. Evidence: 1. In the Mobile Infant Room, there is chipping paint under and near the panda picture. 2. On the toddler playground, the green base of the rocking horse was cracked, creating a potential pinching hazard.

Plan of Correction: The rocking horse was thrown away immediately. The Infant room will be repainted in the next 30 days.

Standard #: 22VAC40-185-500-B
Description: Based on observations, diapering requirements were not met. Evidence: 1. Foot step trash can was not working. It was stuck in the up position in the Toddler Room.

Plan of Correction: It was replaced today.

Standard #: 22VAC40-185-520-C
Description: Based on review of diaper ointments and creams, the center did not obtain written authorization from the parent to apply diaper ointment or cream. Evidence: 1. Child A had a diaper cream with an authorization form that expired on 06/02/2017. 2. Child B had a diaper cream with an authorization form that expired on 08/04/2017.

Plan of Correction: The creams will be sent home today.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on review and interview with the director, one staff member did not have documentation of the central registry results within 30 days and one staff member did not have documentation of the criminal record check results within 30 days. Evidence: 1. Staff A was hired on 05/08/2017. The director documented that the Central registry form was mailed out, but returned due to not being completely filled out. The form was resent, but the date was not documented. 2. Staff B was hired on 08/14/2017. The director documented that the criminal record check was mailed out on 08/16/2017. The director stated Virginia Police was called regarding the document and the document was returned due to incomplete address. It will be resent with the results.

Plan of Correction: I will follow up with both and resend if needed.
