An unannounced monitoring inspection was conducted, October, 12, 2017, from 8:00am-9:15am. A Subsidy Health and Safety inspection was conducted on the same date to review supplemental health and safety requirements. At the start of the inspection, there were 41 school age children directly supervised by 3 staff. The physical plant, 5 staff records, 5 children?s records, and 3 medications were inspected. Children were observed participating in group play, reading books, singing songs, and eating breakfast. There was an abundant supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-280-B Description: Based on observation, the facility did not have hazardous substances such as cleaning materials in aa locked place that prevents access by children. Evidence: 1) In Room A, there were cleaners, disinfectant wipes and a spray cleaner, in an unlocked upper cabinet and disinfectant wipes were on a counter next to the sink at child level. 2) In Room B, there were disinfectant wipes and cleaners in an unlocked lower cabinet. Plan of Correction: In Room A, the wipes were placed in an upper cabinet and the cabinet was locked. In Room B, the lower cabinet was locked.
Standard #: 22VAC40-185-350-E-5 Description: Based on observation, the facility did not maintain proper ratio of one staff member for every 18 children. Evidence: In Room B, Staff Member A was in the room with 23 school age children. Plan of Correction: The staff member moved children into Room A which had two staff member to maintain proper ratio.
