A Subsidy Health and Safety Inspection was conducted on October 13, 2017 to review supplemental health and safety requirements. The Center currently has 44 children enrolled. The children were observed in four classrooms. Staff were engaged with the children. Various activities were observed by the LI in each classroom. Documentation of the center's annual fire and health inspections was current. A sample of 7 children's records was reviewed. Prior to the on-site inspection, complete staff records were reviewed at the central office on September 29, 2017. A licensing inspection was also conducted on the dates of October 12 and October 13, 2017; the two inspections combined cover all subsidy heath and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. This inspection was conducted from 2:00 to 3:15pm. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice. Description: Based on a sample review of 7 children's records, the Center failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. It was determined by review of child #7's record and physical examination form that the child has a diagnosed food allergy. 2. The child's allergy is listed in the center's red book and interviewed classroom staff are aware of the child's allergy. 3. Documentation of a written allergy care plan was not available for review when requested by the LI.

Plan of Correction: The program nurse has made contact with the child's physician and has requested the allergy care plan. Staff in the center are aware of the food allergy and the food is not served to the child. Staff will follow-up with the child's physician.
