22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring visit and facility tour was conducted from 11:40am- 12:49pm. At the time of inspectors' arrival there were 18 pre-school aged children in care with 4 staff members and 1 administrator. Children were observed participating in indoor and outdoor play, lunch, transitioning between activities, and hand washing. A sample of 5 children's records and 8 staff records were reviewed. Injury reports were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.
Standard #: 22VAC40-185-270-A Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There were metal edged cork strips that were along the wall in the early childhood 1 and 3 classrooms t child height that sharp on the ends presenting potential injury hazards. 2. There was a long cord from a radio dangling from the 3' bookshelf that presented potential injury hazards. There were children in care in both of these rooms during this inspection. Plan of Correction: 1. Work order placed to request cap for edge of cork strip. 2. Corrected at time of inspection, cord was wrapped up. Staff will ensure that plugs are covered when not in use.
Standard #: 22VAC40-185-290-3 Description: Based on observation it was determined that the center did not ensure in areas used by preschool aged children all electrical outlets had protective covers. Evidence: There were two uncovered electrical outlets in the early childhood 1 room. Plan of Correction: Corrected at time of inspection. Staff will ensure that plugs are covered when not in use.
Standard #: 22VAC40-185-550-M Description: Based on record review it was determined that the center did not ensure that the written record of children's serious and minor injuries contained all of the required information. Evidence: Three of the eight injury reports reviewed did not contain any future action to prevent recurrence. Plan of Correction: Teachers have been retrained on how to properly complete incident forms.
