Comments:
An unannounced monitoring inspection was conducted with center director and center staff. There were 22 school age children present with two staff supervising. The children were having quiet/homework time, crocheting, and playing in the gym. The last health inspection was conducted 10/19/2016. The last fire inspection was conducted 10/13/2016. Please check with the school office to see if there are updated inspections. Five emergency medications for five children were reviewed and are in compliance. Five children's files were reviewed and are in compliance. One new staff file was reviewed and is in compliance. Description: Based on review of two staff files, the facility failed to have documentation of a repeat search of the central registry (CPS check) for each staff. Evidence: Staff A did not have a repeat search of the central registry (CPS check) on file.

Plan of Correction: Center director will check with the office to see if the CPS check has been mailed and/or returned.
