A Subsidy Health and Safety monitoring inspection was conducted on 10/16/2017 to review supplemental health and safety requirements. This inspection began at 3:45pm and ended at 5:00pm. There were 3 staff with 26 children in 2 classrooms at the time of the visit. At the start of the inspection, children were observed playing in small groups, reading and doing homework. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant was inspected. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today. Description: Based on record review, the program did not obtain a written allergy care plan from a physician for children enrolled with a diagnosed food allergy. Evidence: Child #1 and Child #3 are both diagnosed with food allergies. There is no allergy action plan on site for either child.

Plan of Correction: I will speak to the parents today and ask them to get the form from their physician.
