Comments:
An investigation was conducted today in response to a self-reported incident regarding supervision. Based upon multiple staff interviews and documentation reviewed, the prepoderance of evidence supports a violation. The exit interview was conducted with the Site Director. Inspection conducted from approximately 10:50am to 11:50am. Standard #: 22VAC40-185-340-F
Description: Based upon multiple staff interviews and documents reviewed, A child under age 10 was not kept in actual sight and sound supervision of a staff member. Evidence: 1. On May 26th, 2017 at approximately 3:33pm, Child A (19 months) was not in actual sight and sound supervision of staff for an undetermined amount of time when he exited Classroom 1 unnoticed by staff. Child A ?was [then] seen in hallway by the front desk without a teacher?. 2. Staff A confirmed that she found Child A by the front desk, ?looked down the hallway [and] there was no staff member with him?. 3. Staff B and C confirmed that they heard and saw the classroom door opened by Child A?s brother, and they did not know when Child A left the classroom, and realized he was gone when he came back to the classroom with his mom. 4. Documentation reviewed confirmed that Staff A and B were unaware of Child A?s whereabouts when he left Classroom 1.

Plan of Correction: Early Head Start Site Manager provided additional technical assistance to staff on appropriate supervision and safety measures including all children being within "sight and sound" at all times. Front office staff reminded parent that older siblings must be supervised at all times during arrival and departure to decrease incidents of opening doors. Early Head Start Site Manager reported a plan of correction to the Head Start Coordinator.
