Comments:
An unannounced monitoring inspection was made on 10/10/17. There were 15 children present with three staff supervising. Five children's records were reviewed. The staff records were reviewed on 10/17/17 at the main YMCA site and the inspection was completed and exited with the child care director. No medications were at the site. The inspector arrived at the school at 4:30 pm and departed at 6:00 pm on 10/10/17. The children were observed during outside time and during departures.

Standard #: 22VAC40-185-130-C
Description: Based on record review, the center failed to ensure that one of five children's records had documentation of immunizations required by the State Board of Health. Evidence: 1. The last documentation of immunizations for child #5 were given on 2/15/13. The child would have been less than two years old. The center is required to have documentation of immunizations for children between their fourth and sixth birthday. The child was six years old as of 9/16/17 and was enrolled in public school. The child's first date of attendance at the YMCA after school program was 8/21/17.

Plan of Correction: The staff will have the parent get an updated copy of the child's immunization record.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that one of five children's records contained documentation of required information. Evidence: 1. The record of child #4 had the father listed as an emergency contact person. The Standards require the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Child #4 first date of attendance was 8/14/17.

Plan of Correction: The staff will ask the parent to document another person with all required information as an emergency contact.
