Comments:
The licensing inspector initiated and completed an unannounced complaint inspection in the center on October 12, 2017, in response to a complaint received regarding staffing and supervision. Observations of child to staff ratios were made, interviews with staff members were conducted and center documentation was reviewed. Based on observations, center documentation and interviews conducted during the inspection, a preponderance of evidence does support the allegation regarding staffing and supervision,therefore the complaint is determined to be valid. Areas of noncompliance are noted on the violation notice. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return it to the licensing office within 5 days of receipt. Complaint related: Yes
Description: Based on observation, staff interviews and center documentation, the licensee did not ensure that for children from birth to the age of 16 months there was one staff member for every four children. Evidence: During the inspection on October 12, 2017, it was determined in the infant 2 room from 7:40 a.m. until 8:10 a.m. the ratio of one staff for every four infants was not maintained. At 7:40 a.m. there were eight infants and two staff (staff #11 and staff #12) and the ninth child entered the room at 7:40 a.m. At 7:50 a.m. staff #12 left the room and staff #13 entered the room. The age range of children in the room was from age 12 months (child #4) to age 17 months (child #5). At 8:10 a.m. child #5 left the room and then there were eight children with two staff (staff #11 and staff #13).

Plan of Correction: Staffing schedules and patterns have been changed, effective 10/16/17 to accommodate current needs, in addition new staff are in the hiring process to ensure that all classroom ratios can be met. School management will monitor ratios daily for compliance and DM will randomly monitor for compliance and ensure that no additional enrollment occurs until we can be fully staffed and able to accommodate.

Description: Based on observation, staff interviews and center documentation, the licensee did not ensure that for children from 16 months old to two years there was one staff member for every five children. Evidence: During the inspection on October 12, 2017, it was determined in the toddler 1 room from 7:21 a.m. until 7:45 a.m. the ratio of one staff for every five children was not maintained. At 7:21 a.m. there were five children with only one staff (staff #10) and the sixth child entered the room. At 7:35 a.m. the seventh and eighth child entered the room and there were eight children with one staff. At 7:45 a.m. a second staff (staff #9 ) entered the room and then there were two staff with eight children. The age range of children was 16 months (child #6 ) to age 19 months (child #7).
