Standard #: 22VAC40-185-330-B Description: Based on observations and measurements, the facility did not ensure the required resilient surfacing was located in the required fall zones. Evidence: During the inspection of the playgrounds, a plastic slide that was approximately two and a half feet tall had approximately one inch of pea gravel at the end of the slide. Six inches of pea gravel would be required in all fall zone areas around the slide. Plan of Correction: We will remove that piece of equipment from the playground.
