Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection on Thursday, October 19, 2017 from approximately 11:00 AM to 3:45 PM. The following ratios were observed with a total of 43 children present during the inspection: Infants - 8 children with 2 staff Toddlers and Twos - 8 children with 2 staff Preschool - 11 children with 2 staff Pre-K - 16 children with 2 staff School age children were not present during the inspection. The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Infants were observed playing on the floor with their teachers, sleeping in cribs and being fed. The Toddlers and Twos were observed playing outside and eating lunch. The Preschool and Pre-K children were observed resting at nap time. Lunch consisted of turkey sandwiches, peas, fruit and milk. All areas of the center including classrooms, hallways, playgrounds, and bathrooms were inspected. The facility was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, injury prevention plan, daily sheets, daily attendance, practiced emergency drill logs, emergency preparedness plan, parent agreements, staff training information and liability insurance. The facility?s first aid kit and emergency supplies were inspected. Medication is administered and medication and authorizations forms were reviewed. Transportation is provided and the bus and van along with transportation documented were reviewed. Last documented health inspection: 08/25/2017 Last documented fire inspection: 12/07/2016 During the inspection, five children's records and five staff records were reviewed. Description: Based on observations and measurements, the facility did not ensure the required resilient surfacing was located in the required fall zones. Evidence: During the inspection of the playgrounds, a plastic slide that was approximately two and a half feet tall had approximately one inch of pea gravel at the end of the slide. Six inches of pea gravel would be required in all fall zone areas around the slide.

Plan of Correction: We will remove that piece of equipment from the playground.

Description: Based on observations, the facility did not ensure all prepared infant formula was dated.

During the inspection, the licensing inspector observed three bottles of prepared infant formula located in the refrigerator that were not dated.
