22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:10am-12:25. At the time of entrance, 60 children were in care with 10 staff members present. Children were observed during snack time, circle time, engaged in group activities and outdoor play time. A selection of 6 staff and 6 children records, medications, emergency plan, emergency supplies and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-60-A Description: Based on record review, two of six children's records did not have complete information. Evidence: 1. Child A and Child E were missing documentation of a yearly review of the child's emergency contact information. Plan of Correction: I will show the parents today and have them review and sign.
Standard #: 22VAC40-185-270-A Description: Based on observation, equipment of the center outside was not maintained in a safe and operable condition. Evidence: 1. The orange and blue slide has a large crack on the top left side near where the children place their hands to go down the slide and is a pinching hazard and could entangle clothing or snag skin. Plan of Correction: I will remove the slide immediately.
Standard #: 22VAC40-185-290-3 Description: Based on review, electrical outlets were not all covered with protective covers. Evidence: 1. In the Four's classroom, a surge protector had four outlets that were not covered with protective covers. Plan of Correction: I put protective covers on them immediately.
Standard #: 22VAC40-185-510-N Description: Based on review of medication, when an authorization for medication expires, the center did not dispose of the medication or return to the parent. Evidence: 1. Child E had a medication authorization form for an epi-pen that expired on 07/01/2017. The medication has not been returned to the parent and the authorization has not been renewed. Plan of Correction: I will ask the parents to fill out an authorization form immediately.
