A Subsidy Health and Safety inspection was conducted on 10/19/2017 to review supplemental health and safety requirements. The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Please contact me if you have any questions at Stacy.Doyle@dss.virginia.gov 571-835-0386.
Standard #: VENDSUB-000-004-F Description: Based on review, the center did not post a current list of all children's allergies sensitivities and dietary restrictions. Evidence: 1. In the Four year old classroom, child D was identified with an allergy and the classroom did not have an allergy list posted with parent approval or immediately accessible to staff. 2. In the Two's and Three's classroom, there was not an allergy, sensitivity or diet restriction list. Plan of Correction: I will post the list today.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
