Today an unannounced monitoring inspection was conducted between the hours of 3:20pm and 5:10pm. The Licensing Inspector observed 64 children with 4 staff directly supervising them. Ratios and supervision were in compliance. Children were observed playing on the playground, washing hands, eating snack, doing homework, and playing table games. Staff were actively engaged with the children and assisting them with tasks as needed. Snack served: CapriSun and Baked Cheddar Goldfish. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Medication management was in compliance. Required staff training documents were on-site. Emergency preparedness supplies in order. Required documents posted. Record keeping was in compliance. Standard #: 22VAC40-185-550-L Description: Based on record review, documentation of children's injury reports were found to be incomplete. Evidence: Three of six Injury Report forms reviewed during the inspection were missing the time of parent notification, two of six Injury Report forms were missing the date of parent notification. Plan of Correction: We will have a meeting to go over the correct way to complete the Injury Report forms.
