Reminder: Two shelter-in-place practice drills required annually.

The licensing inspector conducted an unannounced monitoring inspection from 9:55am - 2:00pm. There were thirty one children in care: 5 infants with 2 staff, 6 toddlers with 2 staff, fourteen 2's and 3's with 4 staff, and six 3's and 4's with 2 staff. The children were observed at free play, listening to a story, in group activities, having snacks and playing with play dough. Six staff and five children's records were reviewed. Violations from the previous inspection were also reviewed and three were repeated. Areas of non-compliance are identified on the violation notice. Description: Based on observation, the center failed to maintain the facility in a safe and operable condition. Evidence: 1. The inspector observed the school-age building. There was one toilet in the girl's bathroom that was out of order. There were two seats connected to the lunch table that were cracked and broken with sharp edges.

Plan of Correction: The seats on the table will be covered temporarily and new seats will be made to replace the cracked and broken seats. Maintenance will be completed to repair the toilet.

Description: Based on observation, the center failed to have hazardous substances stored in a locked place. Evidence: The inspector observed a container of Clorox wipes in an unlocked cabinet outside of the boy's bathroom. The inspector also observed a can of Glade spray on the sink and a container of mouth wash on an upper shelf in the staff bathroom. The door to the bathroom was not locked.

Plan of Correction: The containers will be moved to a locked place today.

Description: Based on observation, the center failed to have all nonmedical emergency supplies in the school-age building. Evidence: 1. The inspector observed the emergency supplies in the school-age building. There were no batteries for the radio and no flashlight.

Plan of Correction: A flashlight and batteries for the radio will be placed in the school-age building today.

Description: Based on observation, the center failed to have emergency evacuation and shelter-in-place procedures and maps posted. Evidence: 1. The inspector observed the posted documents in the school-age building. There were no emergency evacuation and shelter-in-place procedures or maps posted.

Plan of Correction: These will be posted today.

Standard #: 22VAC40-185-550-D
Description: Based on record reviews, the center failed to practice monthly evacuation drills. Evidence: The inspector reviewed the documentation for practiced evacuation drills. No drills were documented for the months of April, May, June, July, August or September 2017.

Plan of Correction: The director said additional drills will be practiced and documented to make up for the missing drills. The director will place a reminder on her calendar for monthly evacuation drills.
