Comments:
An unannounced monitoring inspection was completed on 10/11/2017 from 11:50am and ended at 2:30pm. At the end of the inspection there were 107 children in care with 17 staff. Children were observed diaper changing, eating lunch, participating in age appropriate activities and hand washing. The sample size was 10 children's records and 17 staff records where reviewed. The violations are listed on the violation notice and reviewed with the director at the conclusion of the inspection.

Standard #: 22VAC40-185-160-A
Description: Based on record review and interview the provider failed to ensure that the tuberculosis screening form was completed. Evidence: 1 ? The record for staff #6 does not contain documentation of a tuberculosis screening. a. Staff# 6 has a documented date of hire as 4/7/17. b. The provider confirmed that the tuberculosis screening was not completed.

Plan of Correction: Staff # 6 Provided a TB test.

Standard #: 22VAC40-185-70-A
Description: Based upon observation and interview. The facility failed to ensure that background checks as required by the regulation entitled for Licensed Child Day Center staff. Evidence: 1- During the inspection of the facilities staff records. The records for staff # 2 (date of hire 7/24/2017) and staff #7 (date of hire 3/27/17) did not contain documentation of out of state background checks. The facility failed to request background checks from the states of South Carolina (staff #2) and Minnesota (staff#7). 2- During the inspection of the facilities staff records. The records for staff # 2 (date of hire 7/24/2017) did not contain documentation of reference checks and program leader qualifications.

Plan of Correction: Records for staff #2 and staff #7 immediately completed their out of state background checks. A virginia background check was on file for both staff. Staff #2 acknowledged completion of the Primrose learning plan that documented her 34 hours of training upon hiring.
