Comments:
An unannounced renewal inspection was completed on October 20, 2017. The purpose of this visit was to measure compliance with the licensing standards. There were 30 children, infants to preschool age, in care at the time of the inspection. There were 5 staff members working directly with the children in their classrooms. All groups were within the proper ratio. The children throughout the facility were engaged in activity. The infants were crawling on the floor and sitting in infant seats. The toddlers were playing on the floor with their teacher. The preschoolers were having circle time and were reviewing the colors in Spanish. The two year olds were playing with animal figurines and playhouses. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with manipulatives and supplies and items were available to the children. The following information was reviewed: fire and shelter-in-place drills, liability insurance policy, parent agreements, and staff training information. Medication is administered.

Standard #: 22VAC40-185-240-C
Description: Based on a review of a sample of staff records, the facility did not ensure that a staff person completed 16 hours of annual training. EVIDENCE: The record of staff person #5, start date 7/2016, did not have documentation of 16 hours of annual training for the period 7/2016-7/2017. There was only documentation of 14.5 hours of training.

Plan of Correction: The training documentation will be in the staff person's file within 3 weeks.

Standard #: 22VAC40-185-280-B
Description: Based on observations in the infant/toddler room, the facility did not ensure that all hazardous substances were kept in a locked place. EVIDENCE: The inspector observed sanitizing wipes on the half wall in the infant/toddler room.

Plan of Correction: The sanitizing wipes will be removed.

Standard #: 22VAC40-185-500-B
Description: Based on observations at the facility, the facility did not ensure that a nonabsorbent surface for diapering or changing was used. EVIDENCE: The inspector observed a tear in the non absorbent surface covering on the pad used to change diapers in the infant room. The tear exposed fabric that was not nonabsorbent.

Plan of Correction: The pad will be repaired today.
